When you witness or learn
about a suspected fraudulent activity, you should act immediately! Call
the Hotline 24 hours a day at 1-800-468-5469 or contact us
directly (see also Reporting
Fraud).
When you call please
state:

Who
is involved - Give the names of the individual(s),
companies, or other entities involved including addresses or office
locations.

What
is the suspected criminal activity - Give a brief but
accurate statement of the specific facts you believe indicate wrongdoing.

When
and Where did the activity occur - Be as specific as
possible about the location of the incident, and the time(s) the activity
occurred.

How
did you learn about the activity - Tell us if you heard from
someone else, or if you saw what occurred or were asked to perform an act
that struck you as wrong.

Tell
us where you can be contacted - You may report either
anonymously or by providing your identity. It greatly assists
our office's efforts to follow up your report with an interview.
The interview can take place at a mutually convenient time and place.
The department assures employees that no retribution shall be made for their
having made a complaint to OIG, unless the information is willfully false or
misleading. If you wish confidentiality, your identity will not be
disclosed without your consent, and you will be protected to the full extent
of the law.
